Our veterinarians and staff understand how important pets or “furbabies” are to owners.

At Munhall Veterinary Hospital, we always welcome new clients and their pets as well as any new additions to the family. Fill out and submit the appropriate forms below.  New clients are required to pre-pay the exam fee upfront to get on the calendar, this pre-payment will go towards the final balance the day of your appointment. Please call Munhall Veterinary Hospital to pre-pay the exam fee and we can get you scheduled!

If you are a new client, please fill out and submit the New Client Registration Form and the New Patient Registration Form.

If you are an existing client with a new pet, please fill out and submit the New Patient Registration Form.

In order to best serve our clients and their pets, please have your records transferred to the hospital or call us and we can request the records from your previous veterinarian.
